Job Introduction

Home Safe Support Worker - a fixed term 13 month, part time 29.08 hours position, £14.82 - £16.07 per hour. This role is within the Short Term Support Service, providing short term limited interventions to promote people's independence by visiting them in their own homes and supporting them to regain the ability to carry out day to day tasks.

Responsibilities
  • Ensure confidentiality is maintained and each service user is treated with dignity and respect.
  • Develop and follow risk assessments.
  • Plan, implement and review care plans to promote independence.
  • Signpost service users onto other professionals or services.
  • Follow medication guidelines, policies, procedures and audits; maintain accurate and timely documentation.
  • Comply with relevant legislation.
  • Manage a caseload of service users.
  • Use therapy equipment competently.
  • Complete initial visits and home assessments.
  • Interact and engage with colleagues.
Qualifications
  • Valid UK driving licence and access to a car for work purposes.
  • Strong spoken English communication skills.
  • NVQ Level 3 or equivalent, or willingness to work towards an equivalent qualification.
