Social Worker - Hospital Discharge - Royal Borough of Windsor and Maidenhead

Our client, the Royal Borough of Windsor and Maidenhead, is seeking an agency Social Worker for its Hospital Discharge team.

This is a 3-month post initially, paid at up to £35 per hour.

The role will involve the following:

• Provides a range of support and care services for older people, adults with a physical and learning disability and people with mental health needs both in the community and at home.
• Hospital discharge work

Requirements of the role:

• Qualified Social Worker
• Fully Social Work England registered
• Must have significant post-qualified experience in adult social care
• Muat have general hospital discharge experience
