Primary Care Assistant Practitioner, Band 4

An exciting opportunity has arisen for a Primary Care Assistant Practitioner (Band 4) working for Greenwich District Nursing Twi-Light Services.

This is a part time post, 10 hours weekend only, based in the London Borough of Greenwich.

It is essential that you have the Primary Assistant Care Practitioner course completed at foundation degree level or nearing completion.

You will work both on your own and with nurses in the team, visiting patients in their own home; you would be expected to feedback to the team leader during regular handovers. You will have experience of the use of various IT applications including Microsoft and Rio, (Rio training can be provided). Knowledge of personal health and safety responsibilities is essential.
