We are now looking for a dedicated Care Coordinator to join our team someone who is organised, compassionate, and ready to play a key role in delivering outstanding care in the community.

As a Care Coordinator, you will be responsible for planning and coordinating care services, ensuring our clients receive safe, reliable, and high-quality support.

This is a varied and hands-on role, which includes participating in an on-call rota and delivering care when required to support the service.

• Coordinate and schedule care staff to meet client needs

• Ensure care visits are delivered efficiently and on time

• Maintain accurate records and care plans

• Communicate effectively with care staff, clients, and families

• Support with onboarding and ongoing support of care workers

• Participate in the on-call rota to support out-of-hours service delivery

• Deliver care in the community when required to ensure continuity of service

• Ensure compliance with company policies and regulatory standards

• Previous experience in a care or care coordination role

• Strong organisational and problem-solving skills

• Excellent communication and interpersonal skills

• A flexible and hands-on approach

• Ability to work under pressure in a fast-paced environment

• Compassionate and person-centred mindset

• Full UK driving licence (essential)

• Willingness to participate in on-call duties and deliver care when needed
