Brookside Health Centre was established in the mid-1980s and is awelcoming, innovative and progressive practice, working in close partnershipwith other services in the local community and PCN to serve our practicepopulation of 11000+ people.
We are a Limited Company with a GMS contract, and have a dedicated,successful and growing healthcare team.
Main duties of the jobYou will work alongside GP and other colleagues providing full general practice level healthcare to our 11,100 patients, with a proportionate number of patients under your care to support continuity.
Additionally we would like you to take on clinical leadership for a particular area of interest or need within the practice, in time.
Your working week is divided up into various sessions, (morning and/or afternoon) which include F2F and telephone appointments, our Hub telephone triage team, Duty GP cover and Admin.
We have a buddy system in place to ensure support amongst GP colleagues for pathology and correspondence during annual leave, and all GPs provide clinical support and leadership to the rest of the Practice team.
Most of our home visiting is undertaken by our Paramedics and Nurse Consultant, but you would be expected to undertake a home visit as needed, during Duty GP sessions.
The team here is very cohesive and supportive of each other, and everyone is always willing to help when needed, and you would be very welcome to be part of our future.
About usWe have fantastic clinical and administrative support teams here at Brookside.
You will be part of a 6 GP senior clinical team, working alongside 4 Advanced Nurse Practitioners, 3 Paramedics, and a wider team comprising of a Nurse Consultant, Pharmacist, Mental Health Practitioner, Practice Nurses, General Practice Assistants, Social Prescriber, Dietitian, Care Co-ordinator, Health and Wellbeing Coach.
In addition we have a forward thinking Management team and excellent admin support teams, covering all areas.
We pride ourselves as a centre of innovation and excellent care, with a Good overall rating at our last CQC inspection.
