Job Description

Are you passionate about helping people move away from homelessness and towards stability, safety, and independence? We are looking for a dedicated Homelessness Outreach Worker to join a unique team which delivers vital support to individuals sleeping rough or at risk of homelessness. This is a meaningful and fast-paced role where every day you will make a visible difference to people s lives.

About the Role
As a Homelessness Outreach Worker, you will:
  • Provide street-based outreach to rough sleepers across the community.
  • Build trusting and compassionate relationships with people facing homelessness.
  • Carry out assessments and create individualised support plans based on each person s needs.
  • Support individuals to access emergency accommodation, healthcare, benefits, substance misuse services, and long-term housing.
  • Work closely with local authorities, housing partners, health teams, and specialist agencies.
  • Promote safety, wellbeing, and safeguarding at all times.
  • Keep accurate case notes, risk assessments, and monitoring data.
  • This role is varied, rewarding, and essential in supporting people to rebuild their lives.

Experience we re looking for:
  • Empathetic, resilient, and passionate about supporting vulnerable people.
  • Able to work independently in the community and manage a diverse caseload.
  • Confident in building relationships and engaging people with complex needs, including mental health issues, trauma, or substance misuse.
  • Comfortable working with partner agencies and advocating on behalf of clients.
  • Organised, proactive, and calm under pressure.
  • Knowledgeable about homelessness services

Hours & Rota:
The successful candidate will work on a 4 week rolling rota:
  • Week 1: 8am 4:30pm
  • Week 2: 08:30am 17pm
  • Week 3: 8am 16:30 5pm
  • Week 4: 11:30am 8pm
1 weekend per month (9am 1pm) (weekdays off the following week)
