Job Description
We are a training practice with a list size of 17000patients across West Lancs. 2 branches in Skelmersdale, 1 in Southport and 1 in Ormskirk. We are recruiting for a GP to join our team Tuesday, Wednesday and Friday. We have a huge family of clinical staff. We are an innovative practice thatdelivers all national and local enhanced services and offer an abundance oftraining opportunities. We work hard to ensure that our GPs are motivatedand engaged in contributing to a supportive culture of mutual learning. Weenjoy monthly training afternoons for the whole team to come together to share learning creating a collaborative, supportive environment for the entire team. We are purposeful in our effortsto manage GP workload and ensure sustainability, having developed a diverse andlarge team of support staff over recent years dealing with many admin tasks including referral, prescriptions and path links. We have clinicalsupport in-house pharmacists, a nursing and HCA team and our Advanced Practitioners.
We embrace technology to enablemore effective and efficient ways of working. We have introduced an online triage system for our patients allowing our patient to complete an on-line form detailing the problem. It has significantly improved access for our patients and reduced our GP's workload as more conditions can be safely signposted to the appropriate clinician or service.
Main duties of the job Patient first contact is predominantly via Accurx, our on-line triaging system.
1. To provide high quality personal medical services to the practice population, Deliveringsensitive, and responsive care, ensuring theethnic, linguistic and cultural needs are taken into account.
2. To shape and organise services to reflect the needs of the community and practice population
3. To promote the practice as a learning organisation including staying abreast of policy, guidelines and evidence and communicating and integrating this into practice
4. To work effectively within the primary healthcare team, promote team working and take on a leadership role where appropriate
5. To take part in and promote quality improvement processes including clinical governance and audit
6. To work in line with practice aims and objectives

We take on board the Recent BMA guidance in relation to safeworking in general practice and the wellbeing of our workforce, which outlinessafe limits on the number of patient contacts per day.
Our Practice family consists of7 GP's, GP Registrars, 1 ANP, Nurse Prescribers, Practice Nurses,Pharmacists, Community Paramedic, and HCA's. Our administrative team - 17receptionists which includes a head receptionist at each location, 3 clinicalcoders, one scanner, data quality lead, 2 deputy Practice Managers, OfficeManager and Practice Manager. We are an innovative practice that delivers all national and localenhanced services and offer an abundance or training opportunities. We have awhole practice study day every month where the whole of our team gets involved.
In addition to our core general practice work we are a committed member of our local PCN, working Collaboratory to improve patient access within our local area.

1. Role Summary
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all patients.
2. Clinical Responsibilities
In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork (docman) and correspondence in a timely fashion
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Recording clear and contemporaneous consultation notes to agreed standards
Screening patients for disease risk factors and early signs of illness
Developing care plans for health in consultation with patients and in line with current practice disease management protocols
Providing health education
Compiling and issuing computer-generated acute and repeat prescriptions
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a General Practitioner working within primary care.
3. Other Responsibilities
Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, safeguarding, data protection, health and safety etc
Contributing to the development of computer-based patient records in a timely fashion
Coding patient data with applicable codes relating to QOF and practice policies
Contributing to evaluation/audit and clinical standard setting within the organisation
A commitment to life-long learning and audit to ensure evidence-based best practice
Attending training and events organised by the practice or other agencies, where appropriate.
4. Quality
The post-holder will strive to maintain quality within the practice, and will:
Alert other team members to issues of quality and risk
Assess own performance and take accountability for own actions, either directly or under supervision
Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance
Work effectively with individuals in other agencies to meet patients needs
Effectively manage own time, workload and resources
5. Communication
The post-holder should recognise the importance of effective communication within the team and will strive to:

Recognise peoples needs for alternative methods of communication and respond accordingly
6. Safeguarding

The post holder will complete the required level of safeguarding mandatory training, demonstrate a good understanding of safeguarding and will strictly adhere to the Practices safeguarding policies and procedures.

In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
