You will work with clients in their own homes, ensuring they are supported in taking medication, assisting them in activities of daily living, and provide emotional support if necessary, reporting any concerns to the Outreach Manager or the Nurse Manager. Working with qualified nurses as an Outreach Support Worker, you'll recognise indicators of deteriorating mental health and help to implement interventions. Promoting independence, well-being and healthy life choices, you'll also get involved in various activities - from supporting daily living skills following rehabilitation that could include walks, theatre trips, gym sessions and building social networks for clients in the community. With empathy, understanding and patience, we'll not only expect you to love talking to people - but be passionate about helping them too. Having a driving licence is highly desirable.
