About the role:
This is a new role which has the ability to be adapted to meet the need of the service and be flexible to the worker. The role will support with completion of assessments, care and support planning and subsequent reviews both in the hospital and community setting.
About you:
- You will have experience of working in social care or a similar setting with suitable knowledge of the Social Services and Wellbeing (Wales) Act 2014.
- You will be self-motivated, proactive and resourceful and have an ability to create and sustain good working relationships through good communication skills.
- You will be able to work well collaboratively within a team and autonomously, able to recognise when to seek support and to support others.
- You will be able to organise, prioritise and manage your work with good ICT skills including the ability to utilise Microsoft packages and data bases.
- You will have completed a minimum of QCF Level 4 in Care or any other relevant Level 4 social care qualification or have a commitment to complete within 18 months.
What you will do:
- Undertake assessments, care and support planning and reviews which are person centred and outcome focused in both hospital and community settings.
- Understand and utilise the Welsh Government discharge guidance and 'Home First' ethos.
- Work as an integral role within the Hospital Discharge Team who are county wide, to support with non-complex discharges of Powys residents 18+, promoting a strength based and outcome focused approach.
- Work alongside the Hospital Social Workers with reviewing care and support plans post discharge in the community.
