Job Title: Care Navigator - 37.5 hrs per week

Trust Location: Southern Health NHS Foundation Trust

Southern Health NHS Foundation Trust provide community health, specialist mental health and learning disability services for people across the south of England.

They cover Hampshire and one community service on the Isle of Wight.

What you'll be responsible for:

  • To support the planning, co-ordination and transfer of service users across the care pathways.
  • To work in collaboration with the team to support service users as experts in
  • their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.
  • To work in collaboration with other parts of the pathway (e.g. home treatment; statutory AMHP work) to deliver seamless and timely packages of care, geared to the needs of the service user.
  • To develop the role of Care Navigator to embed the Care programme approach within the Mental Health Division.
  • To support clinicians to identify acute care needs such as arranging hospital admission in and out of area.
  • To take the lead in identifying available beds.
  • To support the transfer facilitator in the management of the Bed User Group Meeting.
  • To support the Transfer facilitator to bring back service users who are placed out of area.
  • To work in a manner which is helpful to the service user and the referrer, based on principles of recovery, personalisation, and choice.

You'll learn the following whilst working at the trust:

  • An in depth understanding of the roles and responsibilities involved in working within the NHS
  • Knowledge of the systems used, to effectively complete your role to the highest standard at all times
  • A sense of teamwork, gained through working alongside and supporting colleagues from all levels, within the organisation

You'll have the following skills/experience:

  • System Knowledge: RIO Training & Card
  • Experience in similar role
  • Experience working within the NHS
  • Excellent organisational skills
  • Excellent communication skills
  • Microsoft Office package knowledge e.g., Outlook, Word, Excel
