Service Support Worker -Assisted Discharge Service
Location: Billericay supporting Basildon Hospital and the Red Cross base (covering the South-Essex area)
Contract: Fixed Term
until 31st March 2023 with the potential to extend
Hours: Full-time 35 per week, Monday to Friday 9am to 5pm.
Salary: £18,379 per annum, pro rata

We've got an opportunity...

We are looking for a passionate Service Support Worker to join our South - Essex Support at Home Service. The Independent Living Service Support Worker post will provide reactive, focused and flexible support to inpatients following a hospital stay to enable early discharge when clinical needs have been met. The post will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users' needs in order to re-enable service users to remain in their discharge location and prevent hospital re-admission. Referrals may require the support worker to take the patient home from hospital to enable their discharge.

The post-holder will be required to have the ability to assess service users' needs, complete and follow a support plan for the individual. The post-holder will be providing practical and emotional support in the service user's discharge location and signposting to other services where appropriate to enable the individual to live independently in their accommodation.

The Support Worker will act as contingency for the Service Coordinator when required and may need to provide cover for their colleagues at other hospitals served by the Red Cross in the area. Service confidentiality and safeguarding vulnerable people are key responsibilities of the role.

This post does not include personal care and management of medication.

The Independent Living (IL) & Crisis Response (CR) service (IL/CR)supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care, and emergency services.

This role could be for you if

In order to be successful in your application you must be able to demonstrate experience of working with vulnerable adults. You will also demonstrate that you are a good communicator who can delegate and manage set tasks to agreed timescales. Furthermore, you will have experience preparing and keeping written records and reports. You have ability to work with NHS professionals, community organisations, care agencies and relatives in order to support people in the community to life safely and supported at home.

  • You love being flexible. You sometimes may be required to support at short notice, at weekends etc
Also, you should be the following:
  • IT literate
  • Full driving licence holder (for over 2 years) (essential).
  • Have a good knowledge of services provided by the NHS and Social Care
  • Be local to the area
