x2 Full time Care Assessors postions 37.5 hours

Do you currently work in care as a Homecare Assessor/Team leader? or perhaps never worked as a Homecare Assessor/team leader but always wanted to take the next step?

Car drivers are essential as this role does include visiting clients throughout the day who live in their own homes.

Duties will include:-

Working 37.5hours per week
Undertake assessments whilst involving the Service Users, their families/representatives and other professionals in every stage of the process and to ensure all information is input into the computer system
Making up care plans in readiness for all Service Users care to begin
To ensure understanding of current legislation, local authority requirements and company policy/procedure
Shadowing and supporting of new care worker
Undertake spot checks and audits
Delivering care andsupport to our service users to help promote their independence whilst living in their own home
