Are you an experienced Occupational Therapist looking for your next locum opportunity within a fast-paced community setting?

We are currently recruiting for a Locum Occupational Therapist to join a Discharge to Assess (D2A) service in Cheshire. This is a rewarding opportunity to support patients transitioning from hospital to home, helping to facilitate safe and timely discharges while promoting independence.

• Complete functional assessments in patients' homes following hospital discharge
• Support safe discharge pathways and reduce hospital length of stay
• Recommend and arrange equipment and minor adaptations
• Work closely with MDT colleagues including Physios, Social Workers, and community teams
• Develop patient-centred care plans to maximise independence

• HCPC registered Occupational Therapist
• Experience within community, discharge to assess, or intermediate care settings
• Strong understanding of discharge planning and community rehabilitation
• Ability to work independently and manage a varied caseload
• Full UK driving licence and access to a vehicle (preferred)
