We are looking for a Health and Wellbeing Practitioner to join The Forward Trust at HMP Send, Surrey, working full-time, 37.5 hours per week.

In this role, you ll deliver clinical and psychosocial substance misuse services within a trauma-informed, recovery-focused framework, supporting female service users with substance misuse and low-level mental health needs. You ll help them build healthier futures both inside the prison and as they transition back into the community.

What you ll be doing:

  • Manage your own caseload, delivering 1:1 and group interventions tailored to individual needs
  • Conduct assessments, co-produce care plans, and provide harm reduction advice
  • Facilitate release planning and through-care support for service users returning to the community
  • Work collaboratively with multi-disciplinary teams across mental health, healthcare, and the prison
  • Ensure accurate, timely recording of outcomes and data to track the impact of your work

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
