They work with patients who are homeless or at risk of homelessness. Supporting them to establish accommodation and independent living support solutions on discharge from hospital. Your day to day will involve: Working within hospital settings, in Gloucester and Cheltenham areas, to reduce repeat homelessness and improve outcomes for patients. Supporting the Assistant Team Leader to monitor the quality of support delivered by your team, reviewing impact, and ensuring a psychologically informed approach underpins support work. Achieving individual and team KPI's by supporting the Assistant Team Leader to manage budgets and service contracts, in accordance with all relevant policies and procedures. Working with the Assistant Team Leader to model and embed the culture of continuous improvement within your services. Being responsible for attending and completing mandatory and other required training, development, and online courses About you: Understanding the causes of homelessness and social exclusion for patients and skills which are compatible with delivering a hospital discharge service. Experience of monitoring and reporting on service performance, identifying trends, and proactively steering teams to celebrate success and address gaps in performance. Experience of delivering services in line with quality assurance, health and safety compliance, and data protection legislation and frameworks. Excellent level of IT literacy, operate case management systems and ability to carry out own administrative workload. Highly effective communication skills that allow for effective and clear communication with colleagues and partners at all levels, patients, and all service stakeholders. A full drivers license and access to your own vehicle. All staff have a responsibility to ensure that they develop their personal skills and capacity to meet the changing needs of their role. You will provide reactive, focused and flexible support to adults following a hospital stay to enable early discharge when clinical needs have been met. You will assist the Independent Living Service Coordinators by undertaking duties required to meet the service users' needs in order to re-enable service users to remain in their own home and prevent hospital re-admission. You will be providing practical and emotional support in the service user's own home and signposting to other services where appropriate to enable the individual to live independently in their own home. You can deal with queries in a diplomatic and confidential manner You love being flexible. Working hours out with the norm suits you. Also, you should be the following: Educated to GCSE level (or equivalent by experience) IT literate Full driving licence holder (manual licence essential) Have a good knowledge of services provided by the NHS and Social Care Closing date for application is 23.59 on Sunday the 19th of December 2021 with interviews to follow. This role requires use of full Personal Protective Equipment, including Fluid resistant face masks, to be worn at all times while delivering services. This will be provided for you. The role will involve frontline work with vulnerable services users and will not always allow for a 2 meter social distance to be maintained. You will be asked to complete a Risk assessment to confirm you are able to take up Frontline work, at onboarding stage. If you consider yourself to be a person at greater risk from Covid or you are unable to wear a facemask for long periods of time then this role may not be suitable for you.
