About the role:
You'll be working as part of the Housing on Medical Exit (H.O.M.E) Service, which is funded by Gloucestershire Clinical Commissioning Group for inpatients of Gloucester and Cheltenham hospitals. They work with patients who are homeless or at risk of homelessness. Supporting them to establish accommodation and independent living support solutions on discharge from hospital.
