This will occur within a broad framework of Positive Behaviour Support and Recovery based thinking. - To visit and co-ordinate on going behaviour monitoring of challenging behaviours in relevant CHOICE CARE services. - To assist in the development of interventions aimed to reduce the intensity, frequency and potential challenge of behaviours that are challenging to the individual and the service. - To work with members of support staff to promote independence and the other outcomes as identified in the personalised Positive Behaviour Support Planners. - To offer clearly specified individual and group work to people who receive Choice Care services under the supervision of the Clinical Lead and or the Senior Practitioner. - To facilitate the smooth and efficient implementation of the Care Plan systems within the homes. - To attend reviews, staff meetings, key group meetings, service user meetings etc. as appropriate. - To attend bi-monthly meetings in the services with the Clinical Lead, Senior Practitioner and the manager to review each service user; To provide written reports of reviews and other assessment, formulation and intervention work; To maintain a written report of any individual work. - To act as an information and resource person to staff working at the homes on matters relating to people with learning disability and/ or mental health difficulties. - To participate in the staff training programme for CHOICE CARE staff. - Deliver training for CHOICE CARE GROUP support staff. - Enrol in the training opportunities offered by Choice Care in collaboration with external academic providers. - To carry out literature reviews on relevant topics and to prepare information fact sheets and relevant trainings to be made available to CHOICE CARE GROUP homes. - To undertake research within CHOICE CARE homes as appropriate. The Behaviour Practitioner is responsible to and supervised by the Clinical Lead and the Senior Practitioner. You would be working prominently within the homes and services however working from the Bracknell Head office and from home is also expected from time to time. Ideal Candidates should have experience working with people who have complex needs and a commitment to providing Positive behaviour support, you may also have a degree in Psychology but this is not essential.
