You will be part of the Neighbourhood and Wellbeing Team working in the North and South of the borough. You ll be responsible for: Provide personalised brief intervention and social prescribing support to individuals, their families and carers. Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss what matters to me. Provide person-centred support to enable clients to identify their health and wellbeing needs and goals. Co-produce support plans with clients to discuss what s important, where they would like to be in the future and how we can support them to achieve this. Manage and prioritise a caseload to ensure clients' needs, priorities and urgent support are met. Have a keen interest in the local area to identify gaps in services or community needs and to work with your Neighbourhood team, local service providers and the local community to address these. What you need to bring Able to write compelling communications to advocate for your clients. The Stay Home Safely team are here to support people living in Lambeth that are having to isolate for 10 days due to COVID-19 and offer the help that is needed to stay at home and stay safe. You will be assigned practical tasks that need to be carried out so that clients can safely self isolate at home for 10 days and not have to go out. This could include a number of tasks for that person such as: Collecting and delivering your shopping Collecting and delivering your prescription medicines Delivering ready meals or hot meals Topping up prepayment meters for gas and electricity Dog walking You will be a part of a team that will be carrying out the same tasks and supporting you throughout the day. At the moment we are looking for task workers to cover Friday - Sunday. Please note no tasks will be carried out in the clients home and social distancing is required at all times. PPE is provided and your travel whilst working is also paid for. You need to be happy travelling around the borough all day, confident talking to the public and have good digital skills including using a smartphone, completing online forms and communicating regularly with the team via our online system.
