About this role We are looking for a Lead Worker to join the Community Living and Support Service team. We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions. The Lambeth Living Well Network Alliance supports people in Lambeth experiencing mental illness. Thames Reach works in partnership with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough. You will: Attend ward rounds and daily clinical care meetings to support wards with discharge planning Support patients with home visits / practical tasks Refer and signpost patients to other services Liaise with community mental health teams, social workers, council etc to facilitate safe and timely discharges Keep up to date and accurate records Conduct bed calls and disseminate morning reports when necessary You will have: Experience in multitasking and prioritising competing demands Experience working in a fast paced and demanding role Knowledge and experience of working with vulnerable adults with severe and enduring mental health issues / complex social needs Experience in fostering positive relationships with patients and colleagues Understanding of what constitutes a barrier to discharge from hospital and how to overcome these We are listed in the top 100 UK Best Workplaces 2023 in the UK.
