You will work as part of a multidisciplinary team (MDT) to assist the service in providing outcome based care and support pathways. In this role you will assess, implement and develop an OT treatment programme according to individual's diagnosis and identified needs. This will be based upon findings from assessments and associated risks in order to develop treatment plans & care and support plans. You will also empower staff to improve the quality of care through training provision.
